Which hearing aids provide truly hands-free calling with an iPhone? With or without additional accessory.

The Phonak B-Direct devices as well as the Unitron Moxi All devices provide a true hands-free calling experience with an iPhone with no additional accessories needed. The Oticon OPN, Resound Linx 3D, Signia NX, Starkey iQ, and Widex Evoke devices all need an additional wireless accessory to be truly hands-free. 

So while all major manufacturers offer truly hands free calling with an iPhone, they all do it a little differently. The advantage to the Phonak and Unitron models is that there is no additional accessories needed. The advantage to the Oticon, Resound, Signia, Starkey, and Widex models is that the phone call will stream into both ears (which is necessary for some people due to the degree of their hearing loss and lack of visual cues when using the phone). Your audiologist should be able to help you determine which of these options is best suited to your individual needs.

Phonak & Unitron also allow a direct connection but you only get the call in one ear. One advantage though is you can connect not just with iPhones but Android phones in fact pretty much any Bluetooth equipped mobile phone.

Several brands stream the calls directly to your hearing devices, among them Signia, Oticon, ReSound, & Starkey. So hearing the other party is simple. For them to hear you, however, you either have to hold the phone near your mouth or, like my brother does, put the phone up in your visor while driving. He does that and I can hear him perfectly while he's headed to his office and I'm headed to mine. I can also put my phone upside down in my shirt pocket and people can hear me well.

The Moxi All platform from Unitron allows true hands free calling. The call streams to one ear—specified by the patient. The hearing aid is also the microphone so you do not have to hold the phone to your mouth to talk. You can also answer and disconnect calls with the button on the hear aid. It works with all Bluetooth compatible phones.

All of the big six manufacturers can connect directly to an iPhone.  Starkey, Phonak, Oticon, Resound, Widex and Siemens.
